Welcome to Dutch First’s forms and publications site. The links below provide methods to access and acquire printable copies of our forms (at this time, it is not possible to fill out the forms online). If you cannot find a form you are looking for, please contact us and we will be happy to help.

The following forms are available for download:

Third Party Billing Statement – English Third Party Billing Statement – Dutch	This form is used if a student will not be paying for the language course himself. The form should be downloaded and given to the responsible party (employer, parent, spouse, etc.) to fill out. The TPBS can be returned in person by the student, mailed via post, or scanned and emailed to the school. Read about the rules for the use of the TPBS here.
Accommodation/Housing Application	This form is used if a student who is registered for the One-Year Program wants to request housing through Dutch First. NOTE: Applying for housing through Dutch First has certain restrictions. Read more about them here.
